Physiotherapist

Institution: NHG Population Health

Family Group: Allied Health

Department: Community Health

Join NHG Population Health as a Physiotherapist  and be part of a team that makes a real difference in people’s lives beyond hospital wall. Working within our Community Health team,  you will deliver person-centred, community-based physiotherapy services that helps individuals achieve optimal functioning and meaningful participation in daily life.

This is a role for someone who thrives in dynamic environments, values holistic care, and is passionate about empowering patients within their own communities.

What You Will Do

You will provide clinical care for patients with complex, multifaceted needs — conducting comprehensive biopsychosocial and environmental assessments, formulating clinical diagnoses, and developing individualised, evidence-based therapy plans. Working collaboratively with patients and stakeholders, you will set SMART goals and track progress using appropriate outcome measures.

Beyond direct care, you will have opportunities to shape the broader work of the team. This includes supervising and mentoring junior staff and Allied Health students, contributing to education and training initiatives, and supporting or leading quality improvement and research projects. More experienced candidates may also take on leadership responsibilities such as service planning and interdisciplinary workgroup co-leadership.

As this is a community-based role, you will work across various community posts and conduct home visits, so the ability to travel independently within the community is essential.

*Candidates with more extensive experience and demonstrated senior-level competencies will be considered for a Senior Physiotherapist appointment.
