Why This Role Matters

As a Senior Pharmacist, you will play a critical role in ensuring safe, effective and evidence-based medication management across the continuum of care. Beyond dispensing medications, you will serve as a trusted clinical advisor, educator and leader, partnering closely with healthcare professionals to optimise patient outcomes and elevate pharmacy practice.

Key Responsibilities

Deliver Clinical Excellence

• Review and evaluate medication orders to ensure appropriateness, safety and efficacy.
• Identify and resolve potential medication-related issues, including adverse drug reactions, interactions and prescribing concerns.
• Collaborate proactively with prescribers and multidisciplinary teams to optimise medication therapy and patient outcomes.
• Provide medication counselling and education to patients and caregivers, ensuring safe and effective medication use.

Ensure Safe and Efficient Pharmacy Operations

• Process and fulfil medication orders accurately and within established turnaround times.
• Ensure compliance with all regulatory, legal and institutional requirements relating to medication supply, storage, distribution and record management.
• Maintain accurate documentation and ensure appropriate recovery of medication-related costs and revenue.
• Support operational excellence initiatives that enhance workflow efficiency, service quality and patient experience.

Be a Trusted Medication Expert

• Provide timely, evidence-based drug information and clinical recommendations to healthcare professionals.
• Contribute actively to multidisciplinary discussions, ward rounds, clinical committees and institutional initiatives.
• Support medication safety efforts through investigation, review and continuous improvement activities.

Lead, Coach and Develop Others

• Supervise, mentor and provide guidance to pharmacists, pharmacy technicians and support staff.
• Conduct performance reviews, provide developmental feedback and identify training opportunities for team members.
• Support the training and professional development of pre-registration pharmacists, pharmacy students and pharmacy residents.
• Foster a culture of learning, accountability and continuous improvement within the department.

Drive Innovation and Continuous Improvement

• Review and enhance pharmacy policies, procedures and work processes to improve service delivery and patient safety.
• Participate in institutional and national workgroups, quality improvement projects and strategic initiatives.
• Leverage best practices, emerging evidence and technological advancements to strengthen pharmacy services.
• Pursue continuous professional development and stay current with evolving clinical and regulatory developments.

Contribute to the Hospital Community

• Support organisational initiatives, staff engagement activities, community outreach programmes and quality improvement efforts.
• Actively contribute to fostering a collaborative, patient-centred and high-performing culture across the hospital.

What You Bring

Education & Professional Qualifications

• Bachelor of Science in Pharmacy or equivalent qualification from an accredited institution recognised by the Singapore Pharmacy Council.
• Registered Pharmacist with the Singapore Pharmacy Council.
• Valid Practising Certificate.

Experience

• Relevant experience in hospital, institutional or clinical pharmacy practice.
• Experience in supervising, mentoring or developing pharmacy staff is advantageous.
• Experience in quality improvement, medication safety or service development initiatives is preferred.

• Strong clinical knowledge and medication management expertise.
• Excellent patient counselling and stakeholder engagement skills.
• Ability to interpret clinical information and make sound professional decisions.
• Strong analytical and problem-solving skills, including root cause analysis methodologies.
