MAJOR DUTIES AND RESPONSIBILITIES

 

 

CLINICAL CARE

 

Casework / Psychosocial Care

  • Provide end-to-end case management support and appropriate level of care through assessment, care planning, interventions, and evaluation of complex/specialized cases to achieve positive health and social outcomes for individuals and families.
  • Independently provide clinical interventions for patients and families to support them in the transitions between healthcare institutions and home, empowering them to cope well in the community.

 

Group Work

  • Lead as a subject domain expert in developing, conducting and evaluating group work programmes to build social support systems and networks for target groups.
  • Lead therapeutic group work for target groups.
  • Formulate and outline outreach group programmes for individuals and/or target groups based on outreach strategies.

 

 

 

Community Work

  • Drive engagement with the community through outreach programmes, health education initiatives and preventive strategies to empower individuals and groups.
  • Initiate, design and implement new community development initiatives in specialized areas of health social work.
  • Assist and/or develop and evaluate new service delivery model, programmes and protocols to promote better integration of services within the organization or sector.
  • Deliver public education talks independently.
  • Independently network with targeted community partners for the welfare of individual patients or vulnerable population groups.

 

Programme Development and Implementation

  • Identify service gaps in response to clients' and communities’ needs and make recommendations for improvement of quality and care and service provision to target groups or other stakeholders / partners / agencies.
  • Plan, manage, and deliver programmes for individuals, families, communities and relevant stakeholders in specialized areas of health social work.
  • Participate in reviews of inter-departmental policies and processes.

 

Safe Care

  • Contribute to maintenance and improvement of safe care (e.g. quality improvement projects, adherence to Code of Ethics and hand hygiene, etc).

 

 

education / Clinical supervision

 

  • Supervise interns, support staff and MSWs in their professional development and standards of practice delivery.
  • Seek out continuous learning opportunities within the department / organisation / cluster to further develop skills and capabilities.
  • Develop and conduct specialised workshops/training for Medical Social Workers, healthcare professionals and community partners.
  • Participate in professional sharing sessions to encourage a culture of learning and continuous improvement.
  • Plan and organize education events for patients or public.
