Join our team on a 2-year contract to support community-based frailty screening programmes that empower seniors to age well, stay independent, and access early interventions where needed. Be part of a dedicated team working to improve the health and well-being of seniors through preventive health and community care.
 

Key Responsibilities

  • Coordinate and schedule frailty screening and follow-up assessment appointments for residents.
  • Contact residents via phone calls and SMS for appointment booking, reminders, and follow-up.
  • Educate and encourage seniors to participate in frailty screening and health promotion programmes.
  • Manage screening records, participant databases, appointment tracking, and operational reports.
  • Handle enquiries from residents, community partners, and healthcare stakeholders.
  • Support community outreach activities, health talks, roadshows, and onsite frailty screening operations.
  • Liaise with healthcare providers, community partners, and vendors to ensure smooth programme delivery and follow-up care.
  • Facilitate referrals and follow-up actions for residents who require further assessment or intervention.

 

Requirements

  • Diploma in any discipline.
  • Good communication and interpersonal skills.
  • Comfortable engaging seniors and making outbound calls.
  • Detail-oriented with strong administrative and organisational skills.
  • Proficient in Microsoft Office applications.
  • Able to work weekends and support community outreach or screening events as required.
  • Prior experience in healthcare, community care, customer service, operations, or event coordination will be an advantage.
