Join Our Team as a Senior Social Work Associate!

Principal Responsibilities

• Collaborate with multidisciplinary team to review and refine Recap-lite programme work processes
• Support programme evaluation, outcome measurements, and contribute to publications/conference posters
• Supervise and guide new Recap Coordinators
• Engage adolescent patients and caregivers; conduct biopsychosocial assessments and triage
• Enrol suitable patients and provide psychoeducation, para-counselling, self-help tools and community resources
• Escalate high-risk/complex cases to Doctor/MSW; manage simple casework with community partners
• Initiate clinic outreach and accompany MSW on field visits
• Contact patients and caregivers for appointment reminders and rescheduling
• Document accurately in the Electronic Medical Record system
• Maintain programme databases, work statistics, Eco Map and attend to Recap-lite enquiries
• Perform any other duties as assigned by the MSWs/ MSS-In-Charge

• Willing to travel to NHG Polyclinics when required

Preferred Qualifications!

• At least 4 years of relevant working experience in healthcare/ social services
• Certificate/Diploma in community and social services or equivalent preferred
