Senior Pharmacist, Inpatient & ICU (EG)

JOB PURPOSE

This job description details the responsibilities and the general duties of the Pharmacist (Inpatient and Intensive Care Unit (ICU)).

 

MAJOR DUTIES & RESPONSIBILITIES

Specific

  • Provides pharmaceutical services in WH inpatient acute wards, including ICU. This includes medication reconciliation, timely verification of supplies and discharges for the allocated ward.
  • Participates in multidisciplinary rounds of ICU/acute wards, providing input to ICU physicians on pharmacotherapy. In-house training for ICU coverage can be arranged if needed.
  • Trains Pharmacists and pre-registration Pharmacists to do medication review for these services through a structured framework. 
  • Educates patient on the pharmacological and non-pharmacological aspects of their therapy. Checks compliance and encourages adherence to medication therapy where applicable. 
  • Covers the central discharge pharmacy on a rotational schedule, providing floor support and patient care to ensure timely and safe dispensing of discharge medications.
  • Ensures all sales and supply of medicinal products comply with legal requirements.
  • Ensures records pertaining to all phases of drug receipt, supply, storage and distribution are maintained as required by the hospital and/or legal authorities.
  • Performs inspections of assigned drug storage areas in the hospital.
  • Provides drug information to patients and other healthcare professionals.
  • Responsible for the implementation and adherence to policies, practices and regulations established by the department, hospital and external regulatory agencies pertaining to pharmacy.


General

  • Conduct clinical review for appropriateness of medication orders.
  • Ensure operational efficiency to meet desired turnaround time.
  • Supervise performance and conduct appraisal.
  • Support and actively participate in various activities organised by hospital, such as QC, community projects, staff welfare and recreational activities etc. 


JOB REQUIREMENTS

Education / Training / Experience
i.    Bachelor of Science degree in Pharmacy from an accredited University recognized by the Singapore Pharmacy Council.
ii.    Minimum 2 years Pharmacist Experience
iii.    Prior experience at public health institution.

Professional License 
i.    Registered with the Singapore Pharmacy Council
ii.    Valid practicing license
