STATEMENT OF PURPOSE

 

The Population Health Management (PHM) Directorate has been established to support daily clinical management to become more systematic and targeted; to ensure NHG Health is more informed by clinical, operational and demographic data as Regional Health Manager for Central and North Singapore; and that this informs the way in which we deploy our collective resources across all organisations to maximise the health outcomes of the local population, and establish system planning and care delivery so it is more coherent and systemic in its understanding of population health outcomes, value, and equity.

 

Our role in the PHM Directorate is to facilitate this change across NHG Health at population, provider, place, and resident level – ensuring that we address not just the clinical and preventative aspects of care but also the social, environmental, and behavioral determinants of health – so that we can achieve our ambition to add 15M health years for our residents.

 

This means that the Population Health Management and Value Directorate is principally an ‘enabler’ of this operational and cultural change – developing enabling infrastructure and building new capability at every level of our system in NHG Health on behalf of our partners and population.

 

 

 

MAJOR DUTIES AND RESPONSIBILITIES

 

The executive will support the Population Health Management (PHM) leadership in developing and maintaining comprehensive population health profiles. Their contributions will be essential to improve care quality, outcomes, equity, and optimise system value for defined patient populations and NHG Health residents.

 

The post holder will support the team in the following areas:

  1. Drive population profiling and asset mapping to inform system planning and care delivery to become more systematic and targeted, and
  2. Effective use of population health data to draw insights for strategic planning

 

Key areas of responsibility:

  • Maintain the Knowledge Repository on Population Health’s Sharepoint
  • Refresh and maintain the Population Profiling Dashboard v1.0 and asset mapping files
  • Manage data and/or data analytics requests for PHM

Population Profiling Dashboards

  1. The department maintains two forms of Population Profiling dashboards – interactive dashboards on Tableau and static dashboards saved as PDF files. The executive will provide support to refresh the static profiling dashboards annually.
  2. The executive will provide support to refine the profiling dashboards based on user feedback, refresh with latest datasets and build more dashboards where necessary.

Manage Data and/or Data Analytics requests for PHM

  1. Pop Health staff will submit their Data and/or Data analytics request to PHM via a form.sg. The executive will be in-charge of managing the form.sg submissions and filter the requests appropriately.
    1. Direct requestor to Knowledge Repository where possible
    2. Direct request to PHM analyst
    3. Process straightforward data and/or data analytics requests

  • All form.sg submissions to be handled within 3 working days (i.e. requestors receive an acknowledgement from PHM).

Asset Mapping

  1. The executive will maintain current asset maps for Central and North Singapore made in Google Earth.
  2. The executive will attend to requests for customised asset maps, for example, asset map for POPPIN (a quarterly Pop Health publication).
  3. The executive will maintain the master asset map file that serves as the single source of truth across NHG Health.
  4. The executive will maintain and refresh the NHG Health Postal Code checker based on the master asset map file.

Maintain Knowledge Repository on Population Health’s SharePoint

  1. The Knowledge Repository serves as a one-stop repository for all population health data related resources.
  2. The executive would assist with uploading of materials and refreshing of old content in the knowledge repository.
  3. The executive will assist in enhancing the profiling bot with updated information.
  4. The executive will create awareness of the knowledge repository and other profiling products via eDMs.
  5. The executive would also assist in evaluating the Data Triage workflow that directs staff to the knowledge repository and consolidate areas of improvements based on user’s feedback.

Management of Data Sets and access to data systems

  1. The executive will be the Pop Health Representative for datasets and data systems used across Pop Health, these includes
  1. RHS data package
  2. MOH’s ePlannerX
  1. The executive will have to maintain a list of users who have access to the datasets and cascade updates relating to these data sources from MOH or respective data source teams.

Job Requirements

EDUCATION
·       Bachelor in Mathematics, Public Health, Life Sciences, Statistics or Geography would be preferred.

 

 EXPERIENCE      

  • Relevant experience in population health, healthcare planning, product management or program management roles preferred
  • Working knowledge of Microsoft Office is required and experience in data analytics and visualization tools such as QGIS will be an advantage.

 

 OTHERS               

  • Strong written, analytical and communication skills;
  • Good people management skills and ability to manage multiple projects with effective time management;
  • Meticulous to details;
  • Ability to work well independently and as a team player.
