This role supports the planning and development of Continuing and Integrated Care (CIC) initiatives that bridge hospital care with community services.
You will play a key role in strengthening partnerships, designing care transition programmes, and enabling data‑driven decision‑making to support patients beyond hospital settings. Your work contributes to building healthier communities, improving care continuity, and advancing integrated care models within Woodlands Health.
What You Will Drive
1. Strategic Planning & Partnership Development
- Support the strategic planning and development of Continuing and Integrated Care (CIC) workstreams.
- Foster partnerships with internal stakeholders (e.g. clinical leadership, NHG HQ, Population Health teams) and external community partners.
- Provide secretariat support for committees and workgroups to facilitate alignment and decision‑making.
- Develop proposals and secure funding to pilot and scale key integrated care programmes.
2. Programme Design, Implementation & Care Coordination
- Co‑develop care models and design programmes to strengthen the care transition ecosystem.
- Support implementation of CIC initiatives by coordinating operations, resources, and stakeholder engagement.
- Evaluate requirements across infrastructure, manpower, analytics, and technology to ensure successful programme delivery.
- Provide care coordination support for high‑risk patients requiring transitional care services.
3. Data Management, Monitoring & Reporting
- Conduct data collection and develop monitoring systems to track programme performance and patient outcomes.
- Coordinate reporting requirements for Woodlands Health management, NHG, MOH/MOHT, and other stakeholders.
- Work with relevant stakeholders to ensure timely and accurate submission of data reports.
4. Operations & Administrative Support
- Support bidirectional system access coordination between WH and external partners (e.g. BRIGHT, Partners Linkup).
- Provide administrative and secretariat support for CIC‑related meetings and initiatives.
- Support divisional planning and budgeting processes to align programme implementation with strategic goals.
- Prepare proposal papers and documentation for approvals (e.g. pre‑sourcing, evaluation, award).
- Track timelines and ensure timely follow‑up and closure of key action items.
5. Healthier Communities Initiatives
- Work with Population Health and partners to develop and execute workplans that support patient and caregiver activation.
- Promote health literacy initiatives to improve community wellness across different life stages.
- Plan and execute partner engagement events to strengthen collaboration across the care ecosystem.
- Facilitate connections between internal stakeholders and community partners.
6. Other Responsibilities
- Support additional projects and tasks assigned by the Reporting Officer or Director, Clinical Operations (Integrated Care).
What You Bring
Education & Experience
- University degree in a relevant field.
- Typically 3–5 years of relevant experience in operations, service development, or project management roles for an Assistant Manager position.
- Experience in healthcare, integrated care, or population health initiatives is advantageous.
- Proficient in Microsoft Office applications (Word, Excel, PowerPoint).
