STATEMENT OF PURPOSE |

The role leads the Hospital Associated Disability (HAD) Programme, overseeing the implementation of Person-Centred Humanitude care and the 6Ms framework across General Medical wards. The role combines programme leadership with hands-on clinical coaching, building ward-level capability through mentorship, training, and bedside supervision to ensure consistent, high-quality care for frail older patients and those with dementia. |
MAJOR DUTIES AND RESPONSIBILITIES
(A) SPECIFIC
Programme Leadership and Oversight
· Oversee the end-to-end implementation, progress monitoring, and evaluation of the HAD Programme across inpatient wards, outpatient clinics, community-based care, and home care settings.
· Lead the Humanitude One Care Plan MDT meeting at least weekly to set individualised care goals for eligible patients.
· Facilitate appropriate referrals to the multi-disciplinary team, including Occupational Therapy, Physiotherapy, and Dietetics.
· Manage programme risks, maintain the HAD data registry, and participate in clinical audits, improvement projects, and research to uphold optimal standards in dementia and age-friendly care.
Clinical Coaching and Staff Development
· Serve as the lead Humanitude Coach, providing real-time, hands-on coaching and mentoring to ward nursing and ground staff in the daily practice of Person-Centred Humanitude care and the 6Ms framework (What Matters, Medication, Mentation, Mobility, Multicomplexity, and Meals) at the bedside.
· Oversee integration of Humanitude practices into ward workflows, collaborating closely with nursing and therapy staff to ensure sustainable and effective implementation.
· Facilitate regular peer support groups for ward staff to strengthen team morale and alleviate burnout.
· Support ward-based Coach Nurses in their coaching responsibilities, engaging the Humanitude Trainer where needed to ensure effective transfer of skills.
· Contribute to orientation, performance assessment, and evaluation of nurses in the HAD Programme in partnership with the Ward Nurse Manager.
Patient Screening and Care Delivery
· Supervise and audit daily patient screening to identify eligible patients: age ≥65, presence of dementia or delirium, or behavioural challenges including combativeness, agitation, refusal of care, low mood or motivation, or functional decline or frailty.
· Ensure accurate implementation of the Mentation Care Bundle, including the ARISE workflow (4AT ≥4) and RISE workflow (4AT 1–3).
· Ensure the daily list of eligible patients is shared with the data team IC to facilitate timely data extraction.
· Ensure safe and competent care delivery for all patients under the HAD Programme.
Documentation and Data Quality
· Conduct daily audits of EPIC documentation to ensure accurate and complete data entry by ward staff, covering: This is Me, Johns Hopkins Highest Mobility Scale, Modified Barthel Index, RASS, Intake/Output, Use and Type of Restraint, and 4AT.
Communication and Handover
· Guide shift handovers and nursing reports to ensure accurate communication of 6Ms information, and support effective sharing of care goals during MDT meetings.
Caregiver and Family Support
· Train, support, and counsel family members to equip them to care for patients at home upon discharge, covering Humanitude and person-centred care, nasogastric tube feeding, continence care, and wound dressing.
· Follow up on patients post-discharge through phone calls, home visits, and other appropriate means.
(B) GENERAL
- Attend continuing education, conferences, and seminars to maintain competency in dementia care, Humanitude, and age-friendly care practices.
- Participate in hospital and nursing committees, or take on additional responsibilities as assigned by the Reporting Officer.
