SPECIFIC (80%)

 

  1. Review each patient’s medication chart or prescription for the appropriateness of the drugs prescribed under the supervision of a pharmacist. To contact the prescribing doctor when an intervention is needed. The review must ensure that:
  • All orders are clear and specific.
  • Doses are prescribed within therapeutic limits.
  • Any potential adverse reactions and interactions are identified.

 

  1. Fulfill all drug orders accurately and in a timely manner, under the supervision of a pharmacist.

 

  1. Monitor drug levels (where applicable) and recommend dose adjustments accordingly, under the supervision of a pharmacist.

 

  1. Dispense medications to patients and explain the indications for drug therapy and instructions for administration, under the supervision of a pharmacist.

 

  1. Provide drug information to other healthcare professionals, under the supervision of a pharmacist.

 

  1. Perform sales and supply of medicinal products under the supervision of a pharmacist in accordance with legal requirements:

 

 

(B)       GENERAL (20%)

 

  1. Professional development:
    • Engage in continuous learning through journals and publications to stay current in the field
    • Attend structured learning sessions and participate in audits as assigned.

 

  1. Ensure compliance with departmental, hospital, and external regulatory policies and practices related to pharmacy operations.

 

  1. Complete all Singapore Pharmacy Council mandated training requirements, including workplace-based assessments and portfolio documentation.

 

  1. Actively support hospital initiatives, not limited to:
    • Community Outreach Programmes
    • Staff Welfare Activities
    • Recreational Events
