Pre-Registration Pharmacist
SPECIFIC (80%)
- Review each patient’s medication chart or prescription for the appropriateness of the drugs prescribed under the supervision of a pharmacist. To contact the prescribing doctor when an intervention is needed. The review must ensure that:
- All orders are clear and specific.
- Doses are prescribed within therapeutic limits.
- Any potential adverse reactions and interactions are identified.
- Fulfill all drug orders accurately and in a timely manner, under the supervision of a pharmacist.
- Monitor drug levels (where applicable) and recommend dose adjustments accordingly, under the supervision of a pharmacist.
- Dispense medications to patients and explain the indications for drug therapy and instructions for administration, under the supervision of a pharmacist.
- Provide drug information to other healthcare professionals, under the supervision of a pharmacist.
- Perform sales and supply of medicinal products under the supervision of a pharmacist in accordance with legal requirements:
(B) GENERAL (20%)
- Professional development:
- Engage in continuous learning through journals and publications to stay current in the field
- Attend structured learning sessions and participate in audits as assigned.
- Ensure compliance with departmental, hospital, and external regulatory policies and practices related to pharmacy operations.
- Complete all Singapore Pharmacy Council mandated training requirements, including workplace-based assessments and portfolio documentation.
- Actively support hospital initiatives, not limited to:
- Community Outreach Programmes
- Staff Welfare Activities
- Recreational Events
