Patient Service Associate, KTPH @ Home

MAJOR DUTIES AND RESPONSIBILITIES

(A) SPECIFIC 

  • Manage administrative task related to patient enrolment such as performing Financial Counselling/Onboarding Counselling and billing, including dispensing of Vital Sign Monitoring Kits
  • Schedule home/tele visits and handle phone/email/feedback enquiries from internal and external parties
  • Coordinate post discharge plans such as collection and arranging of medication delivering, sending of discharge summary to patients, scheduling appointment in EPIC and collection of Vital Sign Monitoring Kit
  • Provide guidance and support to patient on self-monitoring devices, including selecting of appropriate blood pressure cuff
  • Assist with coordination with Bed Management Unit for patients requiring diagnostic test/further investigation/care escalation
  • Collaborate closely with doctors, nurses and therapist on issue relating to home visit and patient services
  • Perform daily administrative duties including monitoring the incoming referral list in EPIC and performing actualizing and discharge of patient
  • Perform inventory check on Vital Sign Monitoring Kit
  • Coordinate on settlement of patients with discharge issues with nursing

(B) GENERAL 

  1. Support A & E during exigencies
  2. Work in coordination with other service providers and departments in the discharge of duties
  3. Support corporate activities and community projects
  4. Embrace continuous improvement by suggesting improvement and refining work performance on a daily basis
  5. Assist in other administrative duties as assigned

 

JOB REQUIREMENTS

(A) Education, Training and Experience

  • Minimum Diploma Holder
  • Relevant working experience is preferred
  • Good communication and interpersonal skills
  • Possess positive attitude, initiative and keenness to learn
  • Proficiency in Microsoft office and knowledge of statistics

 

(B) Abnormal Physical Working Condition

  • Be able to respond to emergency recall depending on exigencies of services
