Patient Service Associate, KTPH @ Home
MAJOR DUTIES AND RESPONSIBILITIES
(A) SPECIFIC
- Manage administrative task related to patient enrolment such as performing Financial Counselling/Onboarding Counselling and billing, including dispensing of Vital Sign Monitoring Kits
- Schedule home/tele visits and handle phone/email/feedback enquiries from internal and external parties
- Coordinate post discharge plans such as collection and arranging of medication delivering, sending of discharge summary to patients, scheduling appointment in EPIC and collection of Vital Sign Monitoring Kit
- Provide guidance and support to patient on self-monitoring devices, including selecting of appropriate blood pressure cuff
- Assist with coordination with Bed Management Unit for patients requiring diagnostic test/further investigation/care escalation
- Collaborate closely with doctors, nurses and therapist on issue relating to home visit and patient services
- Perform daily administrative duties including monitoring the incoming referral list in EPIC and performing actualizing and discharge of patient
- Perform inventory check on Vital Sign Monitoring Kit
- Coordinate on settlement of patients with discharge issues with nursing
(B) GENERAL
- Support A & E during exigencies
- Work in coordination with other service providers and departments in the discharge of duties
- Support corporate activities and community projects
- Embrace continuous improvement by suggesting improvement and refining work performance on a daily basis
- Assist in other administrative duties as assigned
JOB REQUIREMENTS
(A) Education, Training and Experience
- Minimum Diploma Holder
- Relevant working experience is preferred
- Good communication and interpersonal skills
- Possess positive attitude, initiative and keenness to learn
- Proficiency in Microsoft office and knowledge of statistics
(B) Abnormal Physical Working Condition
- Be able to respond to emergency recall depending on exigencies of services
