Outreach Coordinator (ENRICH)

As an Outreach Coordinator, you’ll make a tangible difference in patients’ lives by linking clinical care with community resources. Your work will help reduce isolation, support recovery and independence, and enrich the health and resilience of the communities we serve. Join our team and help bridge healthcare and community resources (acitivites) to improve patients’ wellbeing and social connectedness. 

 

This is a contract role until 30 Spetember 2026.

 

What You'll Do:
Patient Engagement

  • Engage patients referred by the care team—either in the polyclinic or by telephone—to introduce and encourage participation in community activities.
  • Present patients with a curated list of nearby community activities, explore their interests, and assist them with signing up.
  • Communicate the benefits of community involvement and motivate patients to participate.
  • Proactively follow up with patients after activities to confirm attendance and gather feedback.
  • Provide way-finding assistance and answer general enquiries about community events and venues.
  • Administer questionnaires to patients and assist with completion when needed to capture outcomes and satisfaction.

Community Partners Engagement

  • Consolidate and maintain an up-to-date schedule of community activities from partners in the polyclinic’s vicinity.
  • Prepare and share patient sign-up sheets with community partners to ensure coordinated tracking.
  • Liaise with partners to obtain verified attendance records for referred patients.

Administrative

  • Maintain accurate records of patients engaged and referred to community activities, ensuring data is confidential and current.
  • Keep the patient’s care team informed about community activities patients are referred to and any follow-up outcomes.

Others

  • Undertake any other duties as assigned by the supervisor that support the objectives of the social prescribing programme and the clinic.

What You'll Need:

  • Diploma level holder and below.
  • Experience in a role involving customer service or direct client-facing responsibilities.
  • Strong interpersonal and communication skills with the ability to engage patients empathetically and professionally.
  • Possess good organisational ability to manage schedules, maintain records and to coordinate with multiple stakeholders.
  • Comfortable in using basic administrative tools and documenting patient interactions accurately. 
  • A team player who can work collaboratively with clinical teams and community partners.
  • Flexibility to carry out duties in-clinic and by telephone, and to adapt to evolving programme needs.
