About the Role
As a Staff Nurse in our Community Health Team, you will work alongside healthcare professionals and community partners to support patients in their homes and neighbours. You will help patients, residents and their caregivers navigate their healthcare journey, promote independence and wellbeing, and contribute to better health outcomes within the community. This role offers valuable exposure to community nursing, care coordination and population health, while allowing you to build meaningful relationships with patients and families.
What You'll Do
- Provide person-centred care and support to patients and residents in the community settings
- Conduct assessments and assist in developing care plans based on patients’ needs
- Support patients and caregivers through education, coaching and guidance
- Work with multidisciplinary teams and community partners to coordinate care and services
- Facilitate smooth transitions of care across different healthcare settings
- Monitor patients’ progress and escalate concerns when required
- Participate in quality and patient safety initiatives
- Participate in training and development opportunities to build your clinical expertise
Who We're Looking For
We are looking for a Registered Nurse with a Diploma or Degree in Nursing with a valid practising licence with the Singapore Nursing Board.
The ideal candidate is passionate about community nursing.
You should possess strong communication and interpersonal skills, be adaptable and resourceful in a dynamic care environment, and have a strong willingness to learn and grow professionally.
