Position: Staff Nurse (1 Year Contract)

Role Description:

Responsible for performing care assessment, planning and management in accordance with established ambulatory nursing policies, standards and evidence-based practices.
Collaborates with the interdisciplinary team to achieve quality holistic care.
Initiates service development and quality improvement projects. Provides patient, family and/or caregiver education as well as promotes health and well-being.
Guides and supervises the support care team, junior staff and students.
Recommends initiatives and implements quality improvement, evidence-based practice or research projects.

General Responsibilities:

Person-centred Care
- Perform biopsychosocial and environmental assessments to identify strengths and needs of patients, families and/or caregivers.
- Communicate with the patient’s healthcare providers, families and/or caregivers to understand patients’ needs.
Patient Engagement and Education
- Involve patients, families, caregivers and/or relevant others as active partners, to identify needs, preferences and expectations of the patients.
- Build therapeutic relationships and maintain professional boundaries when dealing with patients, families and caregivers.
Care Transition and Integration
- Assist patients in navigating care systems based on needs, resources and preferences of patients, families and/or caregivers.
Nursing Practice Management and Operational Excellence
- Assess his/her practices against established organisational guidelines and standards to improve self-efficiency.
- Support quality audits to maintain and improve standards of care.
People and Personal Development
- Attend continuing professional development activities based on learning needs.
- Acquire practice-based learning through applying and reflecting on knowledge and practice.
