Why This Role Matters

Medical Social Workers play a vital role in addressing the psychosocial, emotional, and practical needs of patients and their families—particularly individuals facing complex medical conditions, care transitions, and social vulnerabilities. At Woodlands Hospital, this role is integral to delivering holistic, person-centred care that extends beyond medical treatment to support recovery, resilience, and well-being within the community.

As part of a multidisciplinary healthcare team, the Medical Social Worker facilitates timely intervention, care coordination, and resource linkage, helping to reduce readmissions, strengthen caregiver support, and promote safe discharge planning. Through counselling, advocacy, and collaborative practice, the Medical Social Worker supports WH’s mission to provide seamless, accessible, and compassionate care for a growing and ageing population.

Key Clinical Responsibilities

Casework / Psychosocial Care

  1. The Medical Social Worker will provide holistic case management support and appropriate level of care across the continuum of care services within the campus and into the community through assessment, care planning, interventions and evaluation of care.
  2. The Medical Social Worker is expected to be a member of quality improvement related projects that will improve patient’s outcome.
  3. The Medical Social Worker is expected to meet department clinical indicators.

 

Group Work

  1. The Medical Social Worker will assist in developing, conducting and facilitating group work sessions to establish social support systems and networks for target groups.
  2. The Medical Social Worker will engage individuals and/or target groups who may benefit from group work.
  3. The Medical Social Worker is expected to evaluate group work and articulate outcomes of group work intervention.

 

 

Community Work

  1. The Medical Social Worker will network and collaborate with community partners to conduct needs assessment and implement programs to enhance the psychosocial wellbeing of individual patients and community.

 

Programme Development and Implementation

  1. The Medical Social Worker will participate, implement and assist in the development of programs to improve the quality of care and services.

 

Education / Clinical supervision

  1. The Medical Social Worker will provide guidance, training and co-supervision to Social Work Assistant, Senior Social Work Assistant and Student Medical Social Worker in their standards of practice delivery.
  2. The Medical Social Worker will participate in developmental programs, learning opportunities, and supervision for personal and professional growth and development.
  3. The Medical Social Worker will conduct psychoeducational talks to patients/public with guidance as needed.
  4. The Medical Social Worker will conduct professional workshops and seminars.
  5. The Medical Social Worker will organize, coordinate and provide continuing education within Woodlands Health.
  6. The Medical Social Worker will develop resource materials for education purposes.

 

Research

  1. The Medical Social Worker will apply and demonstrate evidence-based practice.
  2. The Medical Social Worker will critically evaluate current research literature and share with others.
  3. The Medical Social Worker will participate and support research data collection/design/analysis.
  4. The Medical Social Worker is expected to develop abstracts/posters/oral presentations.

 

Management and Leadership

  1. The Medical Social Worker will assist with managing services and resources to achieve targets set by department / Woodlands Health.
  2. The Medical Social Worker will assist in supporting department’s workgroup to deliver and improve service and care.
  3. The Medical Social Worker is expected to participate in department projects.

 

General

  1. The Medical Social Worker is expected to apply social work ethics and values, legislation and international conventions to practice.
  2. The Medical Social Worker is expected to apply strategies and be instrumental to advocate for beneficiaries.
  3. The Medical Social Worker is expected to understand socio-economic-political system within a multi-cultural context and relevant policies.
  4. The Medical Social Worker is expected to be actively involved in department and Woodlands Health’ initiatives.
  5. The Medical Social Worker is expected to support any other duties as assigned by the Supervisor and Chief.

  

What You Bring to the Organisation

Education / Training / Experience

  1. Possess a Graduate Diploma / Degree / Master in Social Work and for fresh graduates, to have completed minimally 400 supervised placement hours.

 

Professional License (if any)

  1. Be a Registered Social Worker (RSW) with the Singapore Association of Social Workers (SASW) or qualify to be a provisional registered social worker.

 

Attributes

  1. Proficient in the use of Microsoft office (Word, Excel, PowerPoint)
  2. Ability to speak dialects, or Malay / Mandarin / Tamil would be preferred
  3. Good communication and inter-personal skills
  4. Knowledge of the social service sector and relevant Public Policies
  5. Need to act with integrity and in accordance to the social work code of ethics
  6. Team-player
  7. Passion in working in the social service sector
