Medical Social Worker, Community Health
JOB PURPOSE
The Medical Social Worker delivers bio-psychosocial-spiritual assessment and intervention to support individuals, families and communities in navigating their healthcare needs. Working closely with multidisciplinary teams and community stakeholders, the incumbent addresses social, emotional and environmental factors that impact well-being, ensuring care is coordinated, holistic and delivered to professional standards of practice.
KEY RESPONSIBILITIES
Clinical Care
Casework & Psychosocial Care
- Provide end-to-end case management through assessment, care planning, intervention and evaluation to achieve positive health and social outcomes for individuals and families.
- Deliver clinical interventions to support patients and families through transitions between healthcare institutions and home, empowering them to cope well in the community.
Group Work
- Develop, conduct and facilitate group work programmes to build social support systems and networks for target groups.
- Collaborate with community stakeholders to identify and reach out to individuals who may benefit from group work.
Community Work
- Engage the community through outreach programmes, health education initiatives and preventive strategies to empower individuals and groups.
- Implement community support systems that enhance the psychosocial well-being of the community, and network with community partners for the welfare of vulnerable individuals and population groups.
- Deliver public education talks with guidance.
Programme Development & Implementation
- Participate in the development and implementation of programmes, and contribute to in-house project work to improve quality of care and services.
Safe Care
- Contribute to the maintenance and improvement of safe care through quality improvement projects and adherence to the Code of Ethics and organisational standards.
Education & Clinical Supervision
- Provide guidance, training and co-supervision to interns and fresh Medical Social Workers in their standards of practice delivery.
- Assist in conducting professional workshops, seminars and continuing education for department colleagues, intra-institutional teams and external partners.
- Deliver education to patients and the public, and assist in developing resource materials for patient care and community health.
- Participate in developmental programmes and supervision for personal and professional growth.
Research
- Apply and demonstrate evidence-based practice, critically evaluating current research literature and sharing findings with others.
- Participate in and support research projects, including data collection, design and analysis.
- Assist in producing academic outputs such as abstracts, posters and oral presentations.
JOB REQUIREMENTS
- A recognised degree or post graduate diploma in Social Work.
- Preferably to be accredited with the Social Work Accreditation Board (SWAB)
