B. PRINCIPAL RESPONSIBILITIES
The Coordinator supporting the Singapore National Asthma Program (SNAP) is responsible to support the care team to optimize the care of asthma patients at NHGP. He/ She ensures patients have regular asthma follow-up and receiving appropriate care; reducing default on appointments; identify and anchor care of asthma patients who are likely to be at high risk for adverse outcomes.
- Patient Care
- Create and maintain asthma registry, including all patients with asthma who have sought care for asthma at polyclinics
- Manage SNAP programme dashboard and act on tasks identified on the dashboard based on established protocols.
- Monitor patients in clinic asthma registry have received asthma counseling
- Monitor patients in clinic asthma registry have asthma symptoms score captured 2 times a year
- Identify high-risk patients, including receiving and anchoring those referred to polyclinic from other settings such as A&E, hospitals, etc
- Ensure regular follow-up of high-risk patients and all asthma registry patients
- Arrange appointments for patients who defaulted their asthma reviews with Doctor or Advanced Practice Nurse or Care Manager
- Identify patients who may require care need assistance and intervene appropriately by alerting respective healthcare team members
- Work with healthcare teams to support and enable healthier behaviors and self-care
- Document care (telephone or face-to-face encounter) provided accurately and concisely
- Collect data that is required for program KPI
- Patient Safety, Quality Improvement and Research
- Comply with guidelines and safe practices according to organizational Work Instructions and Policies
- Apply principles of data protection with access and storage of patients’ information
- Report unsafe practices and incidences per NHGP quality standards
- Participate in quality improvement and/or research activities to improve clinic asthma KPIs
- Training and Development
- Attend appropriate training courses to acquire new knowledge and skills
- Assess own learning needs and undertake learning as appropriate
C. SECONDARY RESPONSIBILITIES
- Participate actively in community outreach and work-based health promotion programs
- Participate and support organizational projects and events
- Perform other duties assigned by the supervisors
