Care Coordinator

NHG Polyclinics is the primary healthcare arm of NHG Health, serving 1.5 million residents across Central and North Singapore.

Responsibilities

  • Identify and engage patients for evidence-based health screenings such as coronary risk and cancer screening
  • Educate patients with chronic diseases on preventive health and diabetes complication screening, including annual foot and eye screening
  • Assist in performing patient assessments including parameters (height, weight, blood pressure), vision testing, Abbreviated Mental Test, Mental Health Assessment and Frailty Screening
  • Obtain social, smoking and alcohol history for patients with chronic diseases during pre-consultation
  • Coordinate patient care with the chronic care team and arrange appointments for follow-up, laboratory tests and workshops
  • Make appropriate referrals for lifestyle modification programmes such as smoking cessation and weight management
  • Teach patients on the use of Health Hub and how to monitor their blood pressure, glucose and weight at home
  • Conduct telephone follow-up with post coronary risk screening patients and offer targeted interventions
  • Support group-based health promotion activities in the clinic and community settings
  • Perform accurate nursing documentation and assist in tracking clinical indicators
  • Adhere to patient safety goals, infection control practices and organisational policies
  • Participate in continuous quality improvement activities

Requirements

  • Minimum GCE O Level; Diploma in Health Management Services and Health Promotion preferred
  • Experience in healthcare preferred; proficient in Microsoft Excel
  • Dynamic, resourceful and proactive with a "can do" attitude
  • Pleasant, cheerful personality with good interpersonal and communication skills
  • A good role model for healthy lifestyle habits
  • Good public speaking and teaching skills will be an advantage
