Responsibilities

• Guide patients in their health journey through behavioural change and motivation
• Bridge care gaps between healthcare providers and community resources
• Create meaningful connections with patients to support their health goal
• Drive better health outcomes through integrated care coordination

Requirements

• Healthcare background with passion for patient advocacy and support
• Strong communication and interpersonal skills
• Ability to build trust and rapport with diverse patient populations
• Self-motivated approach with ability to work independently and as part of a team
• Commitment to continuous learning and professional development
• Tertiary qualifications in counselling, psychology, or behavioural health disciplines are particularly welcome.
