Role: CRC Programme Coordinator (3 years contract)

 

Broad Function 

To provide support on the screening follow-up services for the national colorectal cancer screening programme (Healthier SG screening).

 

Responsibilities:

  • Contact FIT+ patients received from HPB and perform daily phone triaging
  • Schedule appointments and refer FIT+ patients for follow up and colonoscopy
  • Sending SMS to uncontactable FIT+ patients
  • Data entry and reporting of FIT+ patients' follow up outcomes to HPB and for internal tracking
  • Data analysis and preparation of monthly FIT+ statistics to clinicians for tracking of progress under HPB CRC program
  • Reporting and updating of colonoscopy results in hospital system (Epic CRC Tracker)
  • Respond to queries from FIT+ patients/HPB regarding the CRC program
  • Any other duties as assigned by the Reporting Officer

 

Deliverables:

  • Contact residents with FIT+ result, who need further assessment to arrange an appointment within ten (10) calendar days upon receipt of the residents' details from HPB.
  • Ensure that attempts to contact FIT+ residents are done on three (3) separate attempts within a two (2)-week period.
  • Send out phone text messages to remind uncontactable FIT+ residents, which will inform them to return the call and make an appointment.
  • Compile a list of residents with unavailable/incorrect contact numbers and send the list to HPB on a weekly basis.
  • Ensure that FIT+ residents' arranged colonoscopy services are scheduled within twelve (12) weeks from date of HPB referral.
  • Ensure that all required datasets are accurately recorded and submitted to HPB's Screening System on a timely basis.
  • Ensure that required statistics are prepared and circulated to internal stakeholders on a monthly basis.

 

Job Requirements

  • Diploma or GCE ‘A’ Levels preferred
  • At least 3-5 years of related experience in a similar capacity, healthcare experience is preferred. 
  • Able to multi-task and manage projects.
  • Able to communicate effectively at all levels with good interpersonal skills. 
  • Possess a good command of the English language, both written and spoken.
  • Proficiency in Microsoft Office essential
